Can Meditation Really Lower Stress Hormones? What Studies Found

Cortisol is the body’s main stress hormone. Your adrenal glands release it when you’re under pressure, and it’s supposed to drop back down once the pressure passes. “Meditation lowers cortisol” is one of the most repeated claims about meditation. Is it actually true?

The honest answer is: it’s complicated, and the complications are worth understanding, not skipping past.

The Big Picture: A Meta-Analysis

In 2021, researchers Adam Koncz, Zsolt Demetrovics, and Zsofia Takács published a meta-analysis in the journal Health Psychology Review, pooling results from meditation studies that measured cortisol. This wasn’t Vipassana-specific. It combined many types of meditation together.

The finding was more nuanced than a flat “yes.” Using studies that measured cortisol from blood, meditation had a real, medium-sized effect, but only in “at-risk” groups, meaning people already dealing with a physical illness. Using studies that measured cortisol from saliva, a more common and less invasive method, the effect was small and not statistically significant in the general population. In plain terms: meditation may help people whose stress hormones are already elevated by illness, but the evidence is much weaker for otherwise healthy people.

An Intensive Retreat Study (Not Vipassana, But Related)

One of the most detailed cortisol studies came out of what’s called the Shamatha Project, run by researcher Tonya Jacobs and colleagues, published in Health Psychology in 2013. Roughly 60 participants did a 3-month, full-time meditation retreat led by Buddhist scholar B. Alan Wallace. It’s worth being precise here: this was Shamatha, a concentration-focused practice, not Goenka-style Vipassana. The two share some Buddhist roots but are distinct techniques.

The result: people whose self-reported mindfulness increased the most during the retreat also had the biggest drops in their daily cortisol levels. That’s a real, published finding, but it comes from an intensive, 3-month, full-time retreat, a far bigger commitment than a typical daily meditation habit, so it doesn’t tell us much about shorter or more casual practice.

What About Vipassana Specifically?

Here’s where it gets genuinely interesting, and where the simple story falls apart a little. A study in the journal Sleep Science measured cortisol, along with two other hormones, DHEA and melatonin, in 41 long-term Vipassana meditators and 24 non-meditators.

Evening cortisol was about the same in both groups. But morning cortisol was actually higher in the meditators, not lower. The researchers’ interpretation wasn’t “meditation failed.” Instead, they suggested long-term practice may make the body’s whole stress-response system, called the HPA axis, more resilient and better-regulated across the day, rather than just pushing one number down. That’s a more complicated and, frankly, more believable finding than a simple before-and-after drop.

Why the Mixed Results Make Sense

A few real reasons explain the inconsistency across studies. Cortisol naturally spikes right after waking up and falls through the day, so the time of day a sample is taken changes the result. Blood tests and saliva tests don’t always agree. Sample sizes in this field are often small, sometimes 20 to 60 people, which makes it easy for one study to find an effect that another study doesn’t. And three months of full-time retreat practice is a very different exposure than years of daily 20-minute sessions.

Bottom Line

Meditation does appear to affect the body’s stress hormone system, but “lowers cortisol” is an oversimplified way to describe it. The clearest evidence points to benefits for people already under physical stress from illness, and to complex, whole-system changes in long-term practitioners, not a guaranteed drop in stress hormones for everyone who tries it. That pattern of mixed, honest, case-by-case findings also shows up in Vipassana brain research, and in the wider Vipassana science research more broadly.
